Key Responsibilities:
- Community Nursing Care: Provide essential nursing care to patients in their homes, including wound care, administering medications, and managing chronic conditions.
- Patient Assessments: Conduct assessments of patients’ health needs, develop care plans, and monitor their progress.
- Health Education: Educate patients and their families on managing health conditions, medication, and lifestyle changes to improve health outcomes.
- Collaborative Care: Work closely with GPs, other healthcare professionals, and social care teams to deliver integrated care.
- Home Visits: Conduct home visits to assess patients’ living conditions, provide support, and identify any potential health or safety risks.
Qualifications and Requirements:
- NMC Registered Nurse (District Nurse): Must hold a valid nursing qualification and be registered with the Nursing and Midwifery Council (NMC) in the UK.
- Experience in District Nursing: Previous experience as a District Nurse or in community nursing is essential.
- Compassionate Care: Strong interpersonal skills and a compassionate approach to working with patients and their families.
- Communication Skills: Excellent communication skills to interact with patients, families, and the healthcare team.
